Findings: On or about September 22, 2020, Alleged Victim (AV) had an unwitnessed fall in the common area of the facility. An investigation determined that AV is a fall risk and the facility failed to implement reasonable interventions aimed at preventing AV's repeated falls which resulted in AV being diagnosed with a left femoral fracture after being found on the floor. The facility's failure to properly care plan for AV's risk of falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
