- The facility failed to provide appropriate services according to the Alleged Victim's (AV) care plan and needs. The failure resulted in AV going approximately 9 days without bathing, developed skin injury, and was taken to the hospital for evaluation, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
