Findings: On or about September 25, 2020, Alleged Victim (AV) and Witness 1 (W1) had an altercation that caused injury to AV. W1 and AV had an altercation the day prior to this incident. The facility failed to provide a safe environment by failing to implement interventions to protect AV from W1's aggressive behavior. The facility's failure is a violation of resident rights is considered neglect of care and constitutes abuse.
