Findings: The facility failed to provide appropriate supervision and ensure the Alleged Victim's (AV) safety when he/she exhibited signs of exit seeking. The failure resulted in AV eloping the facility and was not discovered until staff were called and notified AV was walking down a busy street, causing risk of serious harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
