Findings: Alleged Victim (AV) relies on the facility to manage his/her medications. On or about September 22, 2020, Alleged Perpetrator 2 (AP2) forged a signature on the narcotics log indicating an as needed narcotic pain medication had been administered at 4:00 pm. The staff signature was of a staff that was not present at the building at 4:00 pm. AP2's actions are considered financial exploitation and constitutes abuse. The facility failed to provide a safe medication administration system, which is a violation of Oregon Administrative Rules.
