Findings: On or about September 30, 2020, Alleged Victim (AV) and Witness 1 (W1) had an altercation in which AV complained of pain following the altercation. W1 has a history of altercations with other residents due to aggressive behaviors. The facility failed to provide a safe environment for AV by failing to protect AV from W1's aggressive behaviors. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
