Findings: On or about October 1, 2020, the Alleged Victim (AV) eloped from the facility through a door that did not properly latch, placing AV at a risk for harm. This is not the first time that AV has eloped from the facility. On this day, Alleged Perpetrator #2 (AP2) was assigned to the unit to care for the residents. During the time in question, AP2 admitted to being on his/her phone in the common area and had no idea that AV had left the building. AP2's actions placed AV at a risk for harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
