Findings: The facility failed to provide a safe medication administration system to ensure the Alleged Victim (AV) received all medications that he/she was supposed to receive. Alleged Perpetrator #2 (AP2) falsified medication records at the facility to show medication had been given to AV as well as other residents when in fact he/she had not given the medication. AV did not have a negative outcome, however, missing medication did not optimize his/her care. AP2's actions are considered neglect of care and constitutes financial abuse. The facility's failures to provide a safe medication administration system and ensure staff oversight are a violation of resident rights, are considered neglect of care and constitute abuse.
