Findings: The Alleged Victim (AV) was care planned to have as needed pain medication to control his/her pain levels. Alleged Perpetrator 2 (AP2) failed to give the medications as needed and requested by AV, leaving AV to experience unreasonable discomfort. AP2 falsified medication records showing that he/she gave the medication to AV when he/she did not. AP2 either threw away the medication or took it home. AP2's actions are considered neglect of care and constitute abuse. The facility failed to provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
