Findings: The facility failed to provide a safe environment for residents during a state of emergency, resulting in a power outage, that left the facility without appropriate staffing, appropriate medication management, fire alarms or emergency lights. During this time, the Alleged Victim (AV) fell and suffered an abrasion to his/her elbow. The facility failed to provide a safe environment for residents during an emergency by not having a plan in place and staff trained how to manage resident care during an emergency. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
