Findings: On or about July 14, 2020, Alleged Victim (AV) and Witness 1 (W1) were involved in an injury altercation. AV and W1 have a history of resident-to-resident altercations and aggressive behaviors. The facility failed to provide a safe environment for AV by failing to protect AV from W1's aggressive behaviors. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
