Findings: Alleged Perpetrator 2 (AP2) failed to administer medications to Alleged Victim (AV) as ordered which resulted in actual physical harm due to medication error causing changes in AV's pulse and respirations. AP2's action is considered neglect of care which constitutes abuse. The facility failed to provide a safe medication administration system to AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
