Findings: On or about October 11, 2020, Alleged Victim (AV) and Witness 2 (W2) had a resident-to-resident altercation in which AV suffered a fall with injury. An investigation determined that W2 showed signs of aggressive behaviors throughout the day prior to W2 assaulting AV. The facility failed to provide a safe environment for AV by failing to protect AV from W2's aggressive behaviors which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-01348 $188.00 fine assessed
