Findings: On or about October 11, 2020, Alleged Victim (AV) and Witness 2 (W2) had a resident-to-resident altercation in which W2 physically assaulted AV. An investigation determined that W2 showed signs of becoming violent and aggressive throughout the day prior to W2 assaulting AV. The facility failed to provide a safe environment for AV by failing to protect AV from W2's aggressive behaviors which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP21-01349 $188.00 fine assessed
