Findings: Witness 1 (W1) has a known history of resident to resident altercations and is care planned for staff to keep him/her six feet from other residents. Alleged Victim (AV) and W1 have had verbal altercations in the past, prior to this incident, in there shared room. On or about October 12, 2020, W1 pushed AV from his/her bed and threw water on him/her, causing AV to have a bruise and trouble moving his/her leg. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
