Findings: On or about August 25, 2020, Alleged Victim (AV) had an unwitnessed fall in his/her room where he/she sustained a head injury and was transported to the hospital for treatment. Facility failed to identify and put interventions into place to address AV's risk for falls. On October 9, 2020, AV had another unwitnessed fall in his/her room and AV was transported to the hospital for treatment and was diagnosed with a hip fracture and underwent surgery. The facility's failure to put person centered fall preventions and/or interventions in place to address AV's fall risk is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-02238 $1500.00 fine assessed
