Findings: The facility and Alleged Perpetrator #2 (AP2) failed to provide a safe medication administration system. The Alleged Victim (AV) was care planned to have oxygen. On or about October 6, 2020, the facility had a power outage occur, leaving AV without oxygen and had to be supplied oxygen by medics. Alleged Perpetrator 2 (AP2) was found to be not substantiated in this case. The facility failed to have a plan in place during a power outage to ensure residents could receive oxygen as needed and failed to provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
