Findings: Alleged Victim (AV) relies on the facility for his/her care. AV receives anticoagulant therapy and should be monitored for bruising or bleeding. On or about August 28, 2020, AV was noted to have bruising to his/her forehead and an abrasion to his/her nose. AV reported to have fallen in the night while toileting. EMS was not called, and AV was not assessed by medical professional immediately after the facility learned of AV’s reported fall and visible injuries. The facility failed to intervene due to AV’s fall and injuries putting AV at risk of serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
