Findings: On or about October 12, 2020, Alleged Victim (AV) and Witness 1 (W1) were involved in a resident-to-resident altercation. An investigation determined that between July 26, 2020 and October 7, 2020, AV was involved in three resident-to-resident altercations and the facility failed to put interventions in place to keep AV safe. The facility failed to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-01882 $375.00 fine assessed
