Findings: Alleged Victim (AV) has a known history of falls. On or about October 5, 2020, staff heard a crash sound and found AV laying on his/her living room floor with blood pooling on the floor from his/her head. AV has had several falls with injuries prior to this incident. The facility failed to properly plan care and mitigate the ongoing risk of injury due to falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP21-02501 $2500.00 fine assessed
