Findings: Alleged Victim (AV) requires facility assistance due to traumatic brain injury/impaired cognition. On or about July 15, 2020. AV's lift chair was left lifted causing AV to slip out of his/her chair. AV expressed a pain after the fall. It is not standard practice to leave the lift chair lifted with a resident in the chair nor is AV able to control the lift chair independently. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP21-01228 $500.00 fine assessed
