Findings: Witness 1 (W1) was care planned to be redirected when attempting to enter other resident’s rooms and had a history of this behavior. On or about October 16, 2020, W1 appears to have attempted to enter Alleged Victim’s (AV’s) room and was pushing the door open as AV was also opening their door resulting in AV falling to the floor and causing AV to get a hip fracture and broken hand. The facility failed to care follow care plan related to W1’s known behavior of wandering, which is a violation of resident rights, is neglect of care and constitutes abuse.
