Findings: On or about October 18, 2020, the Alleged Victim (AV) and Witness #1 (W1) had an altercation that resulted in a skin tear to AV. The facility failed to provide a safe environment for AV, as AV has locked the shared room door in the past which had created arguments between AV and W1. It was foreseeable that an issue would ensue between AV and W1, however, no interventions were put in place to ensure an altercation would not happen. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
