Findings: Alleged Victim (AV) is care planned for stand by assistance with ambulation. On or about October 17, 2020, AV had a witnessed fall. AV was assessed at the time of the fall and had no apparent injury. On or about October 20, 2020, staff discovered AV in bed experiencing pain and had a swollen hip. AV was sent to the emergency room for evaluation and diagnosed with a fractured hip. The facility assumes the fracture was a result from the fall on October 17, 2020. The facility failed to follow AV's care plan which is a violation of resident rights, is neglect of care and constitutes abuse.
