Findings: Alleged Victim (AV) requires staff assistance with transfers and has a known history of getting impatient waiting for staff and self-transferring resulting in his/her falling. AV has had multiple falls. It was reported that staff are slow to respond to call lights. Call light response when the investigator was present was 17 minutes. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
