Findings: On or about October 23, 2020, Alleged Victim (AV) was using his/her wheelchair as a walker, slipped and fell, and sustained bruising and small cuts. An investigation determined that at time of incident, AV was ambulating without proper footwear. AV is care planned to have non-slip footwear when out of bed and proper footwear when ambulating. The facility's failure to provide a safe environment by failing to follow AV's care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
