Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about September 2020, AV weighed 168 pounds. On or about October 2020, AV was documented as weighing 157.2 pounds, a loss of 10.8 pounds, or 6.4% from the prior month. On November 14, 2020, AV’s family contacted the facility with concerns about AV’s weight loss; supplemental protein shakes were initiated on this date and added to AV’s service plan on November 17, 2020. The facility failed to intervene when AV’s condition changed, which is a violation of resident rights, is neglect of care and constitutes abuse.
