Findings: The facility failed to provide basic care, services, and/or safety to Alleged Victim (AV) by failing to provide adequate nutrition to AV which resulted in AV going to hospital for treatment. Between August 4, 2020 and October 27, 2020, AV lost 23.6lb or 14.2% of his/her body weight. The facility's failure to provide oversight and monitoring of change of condition is a violation of resident rights, is considered neglect of care and constitutes abuse.
