Findings: Alleged Victim (AV) is cared planned to be assisted with eating and hydration. AV’s tracked consumption of meals was significantly low. The tracking for fluid intake was not completed most shifts and usually not done in a way to track actual amount consumed. AV was seen in the emergency room on October 28, 2020 and on November 21, 2020 for altered mental status and was determined to be dehydrated both times. The facility failed to follow AV’s care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP21-01634 $250.00 fine assessed
