Findings: On or about November 22, 2020, Alleged Victim (AV) was taken to the hospital with an unexplained scalp wound. An investigation determined that facility staff were conducting inappropriate safety checks by only peeking into AV's bedroom when AV was laying on his/her bed. The facility's failure to provide a safe environment for AV which is a violation of resident rights, is considered neglect of care and constitutes abuse.
