Findings: The facility failed to follow Alleged Victim's (AV) care plan by not changing AV's catheter which put AV at risk of harm. An investigation determined that home health was cancelled on November 11, 2020 due to facility not responding to multiple inquires about AV's catheter care. In December of 2020, the last documented catheter change was September 14, 2020. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
