Findings: Alleged Victim (AV) has a known history of falls. Between October 1, through November 30, 2020, AV sustained approximately eighteen (18) falls. On or about December 1, 2020, AV was found to have bruising on both of his/her hands; AV was protecting his/her left hand and would not use it. AV went to urgent care on December 2, 2020 and was found to have two fractures on his/her left thumb. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP21-03081 $1500.00 fine assessed
