Findings: On or about December 3, 2020, Alleged Victim (AV) fell out of his/her bed and suffered pain and bruising. AV has a history of falls. An investigation determined that AV suffered from at least eleven (11) falls in the last year and the last fall out of his/her bed was July 17, 2020. The facility's failure to develop or maintain a comprehensive care plan for AV to prevent future falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
