Findings: The facility failed to provide basic care, services, and safety for Alleged Victim (AV) which resulted in AV having an unwitnessed fall on or about December 2, 2020, in which AV was sent to the hospital for treatment and was diagnosed with several broken ribs and a collapsed lung. An investigation determined that AV experienced 13 documented falls between August 26, 2020 through December 2, 2020 and the facility failed to implement reasonable interventions aimed at mitigating or preventing AV's frequent falls. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
