- Findings
- The facility failed to provide a safe environment and appropriately monitor Witness 1 and the Alleged Victim’s (AV) according to their known behaviors and prior altercations. The failure resulted in a physical altercation, causing unreasonable discomfort to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
