Findings: Alleged Victim (AV) and Witness 1 (W1) are known to go into each other’s rooms. On or about December 1, 2020, W1 entered AV’s room on two separate occasions, in the same day. On the first occasion, W1 was found standing over AV while AV was on the floor with an injury to his/her head. On the second occasion, W1 was found lying on AV’s bed and AV was found lying on the floor. The facility failed to care plan related to W1’s known confusion of rooms, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP21-02166 $500.00 fine assessed
