Findings: On or about December 4, 2020, Alleged Victim (AV) and Witness 2 (W2) had a resident-to-resident altercation in which W2 hit AV with a closed fist. The facility failed to provide AV with a safe environment by failing to protect AV from W2's aggressive behaviors which is a violation of resident rights, is considered neglect of care and constitutes abuse.
