- Findings
- On or about November 30, 2020, Alleged Victim (AV) fell in his/her bedroom near the bathroom resulting in a skin tear on his/her head. An investigation determined that AV had five (5) unwitnessed falls in his/her bathroom between September 20, 2020 and November 30, 2020 of which AV sustained harm or injury. The facility failed to provide assistance with toileting and supervision for safety resulting in AV falling multiple times and sustaining physical harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
