Findings: On or about December 6, 2020, the Alleged Victim (AV) suffered a fall from his/her wheelchair, causing a hematoma, cuts and rug burns. AV has fallen from his/her wheelchair before and it was determined prior that AV should have a tilted wheelchair to prevent falls. The facility never ordered this chair until after this most recent fall. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-01879 $188.00 fine assessed
