Findings: Alleged Victim (AV) had a known history of wandering behavior that resulted in altercations with other residents. On or about December 8, 2020, AV attempted to enter Witness 1 (W1) and Witness 4 (W4)'s room, resulting in W1 and W4 attempting to forcefully stop AV from entering the room. Evidence suggest that AV was struck in the head with a potted plant as evident by a broken potted plant on the floor and AV was found with dirt in his/her hair. AV reported pain to his/her foot and head following the incident and it took approximately eighty two (82) minutes for AV to calm down. AV was not assessed by a medical professional after the incident nor was his/her vitals taken. The facility failed to care plan regarding AV's known behavior of wandering, which is a violation of resident rights, is neglect of care and constitutes abuse.
