- The facility failed to ensure that Alleged Victim (AV) was administered his/her anticoagulant medication as ordered which resulted in AV missing seven (7) consecutive doses which placed AV at risk of harm. The facility's failure to provide a safe medication administration system is a violation of resident rights, is considered neglect of care and constitutes abuse.
