Findings: Alleged Victim (AV) had a known history of behaviors requiring staff interventions to mitigate AV's risk of negative outcomes due to diagnosis of dementia. On or about November 25, 2020, AV was involved in a resident to resident altercation and when staff attempted to intervene the outcome was AV becoming physically aggressive with staff. The facility failed to care plan and give staff clear direction on AV's behaviors, interventions and how to protect AV and other residents from a negative outcome putting AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP21-02601 $500.00 fine assessed
