Findings: The facility failed to provide a safe environment for Alleged Victim (AV) by failing to protect AV from Witness 1's (W1) sexual advances. On or about December 21, 2020, W1 was found in AV's room wearing no pants and acting in an inappropriate sexual manner. An investigation determined that W1 has a history of sexual and physical aggression towards AV and others. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-02385 $500.00 fine assessed
