Findings: On or about November 29, 2020, Alleged Perpetrator 2 (AP2) left Alleged Victim (AV) sitting alone in his/her wheelchair resulting in AV falling out of his/her wheelchair and sustaining a laceration on his/her head. AP2's actions is considered neglect of care and constitutes abuse. The facility failed to provide a safe environment for AV resulting in AV falling multiple times. An investigation determined there were no interventions documented or formally implemented after any of AV's falls from October 2020 through November 2020. Although it was a known protocol to not leave AV alone in his/her wheelchair due to his/her risk of falling, the facility did not provide staff with documented instructions to implement this protocol. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
