Findings: The facility failed to implement interventions and appropriately monitor the Alleged Victim (AV) according to his/her fall history and needs. The failure resulted in AV experience an unwitnessed fall with injury and was on the floor for an undetermined period of time before being found by staff, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
