Findings: The Alleged Victim (AV) was care planned to have his/her toenails trimmed two times per week after showers. On or about December 25, 2020, AV's family took AV home for a visit and discovered that AV's toenails were about an inch overgrown, causing unreasonable discomfort for AV. The facility failed to follow AV's care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
