- Findings
- Alleged Victim (AV) was a known fall risk, had a history of not wanted to wait for staff assistance with transfers, and informing the facility several days after AV experiences a fall. On or about December 28, 2020, AV fell resulting in a fractured pelvis. The facility failed to care plan and implement interventions to mitigate AV's risk of injuries due to falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
