Findings: On or about December 19, 2020, it was discovered that the Alleged Victim (AV) had bruising on his/her right wrist and hand, which was large and worse than "typical" bruising found on AV, as he/she bruises easily. AV stated staff were being rough with AV and caused the bruising. Alleged Perpetrator 2 was found to not have committed any abuse or wrongdoing in this case. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
