Findings: On or about December 31, 2020, it was discovered that the Alleged Victim (AV) had unexplained bruising to his/her knee. The facility failed to investigate the cause of AV's repeated bruising and put meaningful interventions in place to prevent further bruising. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
