Findings: On or about January 3, 2021, the Alleged Victim (AV) exited the facility without assistance. AV was out of the facility approximately 2 hours and was found outside the facility in the berry bushes, with scratches and bruises. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP21-02225 $188.00 fine assessed
